Summary

Correct the fallback getLocFromIndex implementation used when a language's SourceCode object does not provide that method.

The cached positions currently point at the newline characters themselves, rather than the start of the following line. For example, the b in a\nb is reported at line 2, column 1 instead of column 0; CRLF produces column 2. Standalone carriage returns and Unicode line separators are not recognized.

Cache actual line-start offsets, handling the same LF, CRLF, CR, U+2028, and U+2029 line endings as ESLint's native source locations. Locate the containing line by binary search instead of scanning from the first line for every report. Keep the WeakMap cache and native sourceCode.getLocFromIndex delegation unchanged.

Verification

• pnpm install --frozen-lockfile passed.
• pnpm test passed lint and all 38 existing tests, including the @eslint/json cases.
• An inline comparison checked every offset through EOF against ESLint's native SourceCode method for 20 inputs: empty text, ordinary lines, empty first/last lines, repeated empty lines, and five newline forms. Upstream mismatched 60 of 100 offsets; the patch matched all 100. Native-method delegation was also verified.
• An illustrative cached-lookup benchmark with 20,001 lines and 10,000 lookups took 33.61 ms before and 10.76 ms after on the local machine. This isolates the helper and is not a claim about whole-project lint speed.
• No existing tests were modified and no new tests were committed; the focused comparison and benchmark ran inline.

Compatibility

No public API, dependency, formatter, or autofix-output changes. Fallback diagnostic line/column positions are corrected; source objects supplying their own method follow the existing path.
