feat(apple): finish with content-filter for guardrails and refusals - #321
Through the AI SDK, GUARDRAIL_VIOLATION and REFUSAL no longer throw. doGenerate returns no content and doStream ends with a finish part, both with finishReason: { unified: 'content-filter', raw: 'GUARDRAIL_VIOLATION' | 'REFUSAL' }, which is how other providers report content filters. Text streamed before the guardrail tripped is kept. The low-level AppleFoundationModels API still rejects with the code.
Behavior change: callers that caught these errors from generateText/streamText should check finishReason instead. Before #320 they were GENERATION_ERROR.
Harness tests cover doGenerate, doStream and the native API with a prompt the guardrails flag. Generate and stream tests pass on the iPhone 18 Pro simulator (iOS 27).
Guardrail violations and model refusals now end AI SDK responses with a content-filter finish reason instead of failing generation. Non-streaming responses return no content; streaming responses preserve text already delivered but do not emit held structured output after filtering. Responses include the raw reason when available. Other errors continue to follow existing error handling.
Documentation
Updated guidance to explain content-filter responses, streaming behavior, and raw reasons. The low-level Apple Foundation Models API continues to reject requests with guardrail or refusal errors.
Guardrail violations and refusals failed the call. Through the AI SDK they
now end the response with a content-filter finish reason, like other
providers' content filters. The low-level API still rejects with the code.

Walkthrough
The Apple AI SDK adapter converts native guardrail violations and refusals into content-filter finish results for generation and streaming. Harness tests cover these behaviors and structured-output handling. Apple documentation describes the finish reasons and low-level API errors.
Changes: Apple content-filter handling

Summary
Map native filter errors to finishes packages/apple-llm/src/ai-sdk.ts
Generation returns empty content for guardrail violations and refusals. Streaming emits a content-filter finish, avoids flushing held structured-output text, and closes. Other errors retain their existing handling.
Cover and document filter handling harness/apple/generate.harness.ts, harness/apple/stream.harness.ts, website/src/docs/apple/generating.md
Harness tests cover guardrail handling in generation and streaming, including structured output. The documentation describes content-filter finishes, raw codes, and low-level API rejections.

Before / after behavior
observed — Modified behavior in website/src/docs/apple/generating.md: The Error Handling section now documents that guardrail violations and refusals end AI SDK responses as content-filtered, preserve previously streamed text, and use GUARDRAIL_VIOLATION or REFUSAL as raw values; low-level AppleFoundationModels calls reject with those codes.
observed — Modified behavior in harness/apple/generate.harness.ts: The imports add AppleFoundationModels while retaining apple and createAppleProvider.
observed — Modified behavior in harness/apple/generate.harness.ts: A shared flaggedPrompt string is added for the guardrail-related tests.
observed — Modified behavior in harness/apple/generate.harness.ts: A test now checks that apple().doGenerate returns empty content and a content-filter finish reason with raw value GUARDRAIL_VIOLATION for the flagged prompt.

Add deterministic coverage for buffered structured-output suppression.
If native guardrails reject the prompt before the first update, this integration test passes without exercising end({ flushHeldText: false }). Add a deterministic createSnapshotStreamHandler test that first supplies nonempty structured output, calls end({ flushHeldText: false }), and asserts that no text-delta is emitted. Keep this test for native integration coverage.

Addressed CodeRabbit's nitpick: added a deterministic harness test that feeds createSnapshotStreamHandler a structured snapshot and checks end() sends it while end({ flushHeldText: false }) sends nothing, so it doesn't depend on when native guardrails fire.
